​CHOSEN CDC is a civic outreach organization focused on addressing the immediate needs and to promote the sustainment of healthy lives in our community!
We are laser focused on programs that promote community health, welfare and readiness. The CCDC relies on cooperative engagement with you, our civic leaders and local community stakeholders to provide solutions that facilitates and drives self-empowerment, health, wellness, personal growth and development. CCDC is also a champion and strong advocate to strengthen the impact and outcomes of a community education, outreach and advocacy programs.
